You have two options for submitting the Mentor and Mentee Forms:

- Download and fill out the Mentor or Mentee Forms (below), which can submitted by:
- Mentor Form (Adobe PDF | MS Word)
- Mentee Form (Adobe PDF | MS Word)
- E-mail the completed form to mamart3@email.uky.edu
- Fax the completed form to (859) 257-7794
- Mail the completed form to:
